What Other Diseases Masquerade as Rheumatoid Arthritis? Part 1 - The Non-Infectious Group


Rheumatoid arthritis (RA) is the most common form of inflammatory arthritis and affects more than 2 million Americans. The diagnosis is not easy to make in many instances. There are more than 100 different kinds of arthritis. Most of them involve inflammation. When a patient goes to a rheumatologist to get a diagnosis, there is a process of elimination in order to arrive at the proper diagnosis. This process of elimination is called "differential diagnosis."

Differential diagnosis can be a difficult undertaking because so many forms of arthritis, particularly inflammatory forms of arthritis look alike. Generally it is helpful to divide the differential diagnosis of rheumatoid arthritis into two groups. The first group are the non-infectious diseases to consider and the second group are the infection-related conditions.

Since the discussion is rather long I have chosen to divide the article into two parts.

The following is a partial list of forms of inflammatory arthritis that can be seen and must be considered when evaluating a patient with inflammatory symptoms of arthritis and are not infection related.

RA is an autoimmune chronic inflammatory disease, primarily involving the peripheral joints (hands, wrists, elbows, shoulders, hips, knees, ankles, and feet). It can also affect non joint structures such as the lung, eye, skin, and cardiovascular system.

RA may start slowly with nonspecific symptoms, including fatigue, malaise (feeling "blah"), appetite loss, low-grade fever, weight loss, and vague joint pains, or it may have an explosive onset with inflammation involving multiple joints. The joint symptoms usually occur bilaterally- both sides of the body equally involved- and symmetric. Erosions- damage to the joint- can be seen with x-ray. In about 80% of cases, elevated levels of rheumatoid factor (RF) or anti-cyclic citrullinated antibodies (anti-CCP) are present in the blood. There appears to be a correlation between the presence of anti-CCP antibodies and erosions.

Juvenile rheumatoid arthritis (JRA) occurs in children under the age of 16. Three forms of JRA exist, including oligoarticular (1-4 joints), polyarticular (more than 4 joints), and systemic-onset or Still's disease. The latter condition is associated with systemic symptoms -- including fever and rash in addition to joint disease.

Polyarticular JRA has similar characteristics to adult RA. It causes about 30% of cases of JRA. Most children with polyarticular JRA are negative for RF and their prognosis is usually good.

Approximately 20% of polyarticular JRA patients have elevated RF, and these patients are at risk for chronic, progressive joint damage.

Eye involvement in the form of inflammation- called uveitis- is a common finding in oligoarticular JRA, especially in patients who are positive for anti-nuclear antibody (ANA), a blood test that is often used to screen for autoimmune disease. Uveitis may not cause symptoms so careful screening should be performed in these patients.

SLE is an inflammatory, chronic, autoimmune disorder that can involve the skin, joints, kidneys, central nervous system, and blood vessel walls. Patients may present with 1 or more of the following: butterfly-shaped rash on the face, affecting the cheeks; rash on other parts of the body; sensitivity to sunlight; mouth sores; joint inflammation; fluid around the lungs, heart, or other organs; kidney abnormalities; low white blood cell count, low red blood cell count, or low platelet count; nerve or brain inflammation; positive results of a blood test for ANA; positive results of a blood test for antibodies to double-stranded DNA or other antibodies.

Patients with lupus can have significant inflammatory arthritis. As a result, lupus can be difficult to distinguish from RA, especially if other features of lupus are not present. Clues that favor a diagnosis of RA over lupus in a patient presenting with arthritis affecting multiple joints include lack of lupus features, erosions (joint damage) seen on x-rays, and elevations of RF and anti-CCP antibodies.

Polymyositis (PM) and dermatomyositis (DM) are types of inflammatory muscle disease. These conditions typically present with bilateral (both sides involved) large muscle weakness. In the case of DM, rash is present. Diagnosis consists of finding the following: elevation of muscle enzyme levels in the blood [the two enzymes that are measured are creatine kinase (CPK) and aldolase], signs and symptoms, electromyograph (EMG)- an electrical test- alteration, and a positive muscle biopsy.

In addition, in many cases abnormal antibodies specific for inflammatory muscle disease can be elevated.

In both PM and DM, inflammatory arthritis can be present and can look like RA. Both inflammatory muscle disease and RA can affect the lungs. In RA, muscle function will usually be normal. Also, in PM and DM, erosive joint disease is unlikely. RF and anti-CCP antibodies are typically elevated in RA but not PM or DM.

SAs -- psoriatic arthritis, reactive arthritis, ankylosing spondylitis, and enteropathic arthritis -- are a category of diseases that cause systemic inflammation, and preferentially attack parts of the spine and other joints where tendons attach to bones. They also can cause pain and stiffness in the neck, upper and lower back, tendonitis, bursitis, heel pain, and fatigue. They are termed "seronegative" types of arthritis. The term 'seronegative' means that testing for rheumatoid factor is negative. Symptoms of adult SAs include:

o Back and/or joint pain;

o Morning stiffness;

o Tenderness near bones;

o Sores on the skin;

o Inflammation of the joints on both sides of the body;

o Skin or mouth ulcers;

o Rash on the bottom of the feet; and

o Eye inflammation.

Occasionally, arthritis similar to that seen in RA can be present. Careful history and physical examination can often distinguish between these conditions, especially if an obvious disease that is promoting inflammation is present (psoriasis, inflammatory bowel disease, etc.). In addition, RA rarely affects the DIP joints- the last row of finger joints. If these joints are involved with inflammatory arthritis, the diagnosis of an SA is possible. (Note of caution: a condition known as inflammatory erosive nodal osteoarthritis can also affect the DIP joints). RF and anti-CCP antibodies are negative in SAs, although, rarely, in cases of psoriatic arthritis there may be elevations of RF and anti-CCP antibodies.

Gout is caused by deposits of monosodium urate (uric acid) crystals into a joint. Gouty arthritis is acute in onset, very painful, with signs of significant inflammation on exam (red, warm, swollen joints). Gout can affect almost any joint in the body, but typically affects cooler areas including the toes, feet, ankles, knees, and hands. Diagnosis is made by drawing fluid from an inflamed joint and analyzing the fluid. Demonstrating monosodium urate crystals in the joint fluid is diagnostic, although finding elevated serum levels of uric acid can also be helpful.

In most cases, gout is an acute single joint disease that is easy to distinguish from RA. However, in some cases, chronic erosive joint inflammation where multiple joints are involved can develop. And, in cases where tophi (deposits of uric acid) are present, it can be difficult to distinguish from erosive RA. However, crystal analysis of joints or tophi and blood tests should be helpful in distinguishing gout from RA.

Calcium pyrophosphate deposition disease (CPPD), also known as pseudogout, is a disease is caused by deposits of calcium pyrophosphate dihydrate crystals in a joint. The presence of these crystals in the joints leads to significant inflammation. Establishing the diagnosis includes using:

o Detailed medical history;

o Withdrawing fluid from a joint to check for crystals;

o Joint x-rays to show crystals deposition in the cartilage (chondrocalcinosis); and

o Blood tests to rule out other diseases (e.g., RA or osteoarthritis).

In most cases, CPPD arthritis presents with single joint inflammation. In some cases, CPPD disease can present with chronic symmetric multiple joint erosive arthritis similar to RA. RA and CPPD disease can usually be told apart by joint aspiration demonstrating calcium pyrophosphate crystals, and by blood tests, including RF and anti-CCP antibodies, which are usually negative in CCPD arthritis. A complicating feature is that RA and CPPD can coexist!

Sarcoidosis is an inflammatory joint disorder. The majority of patients with this disease have lung disease, with eye and skin disease being the next most frequent signs of disease. Although the diagnosis of sarcoidosis can be made on clinical and x-ray presentation alone, sometimes the use of tissue biopsy with the demonstration of "noncaseating granulomas" is necessary for diagnosis.

Arthritis is present in 15% of patients with sarcoidosis, and in rare cases can be the only sign of disease. In acute sarcoid arthritis, joint disease is usually of rapid onset. It is symmetric involving the ankles, although knees, wrists, and hands can be involved. In most cases of acute disease, lung and skin disease are also present. Chronic sarcoid arthritis can be difficult to distinguish from RA. Although RA-specific blood tests, such as RF and anti-CCP antibodies, can be helpful in distinguishing RA from sarcoidosis, in some cases a biopsy of joint tissue may be required for diagnosis.

Polymyalgia Rheumatica (PMR) is a disease that leads to inflammation of tendons, muscles, ligaments, and tissues around the joints. It presents with large muscle pain, aching, morning stiffness, fatigue, and in some cases, fever. It can be associated with temporal arteritis (TA), also known as giant-cell arteritis, which is a related but more serious condition in which inflammation of large blood vessels can lead to blindness and aneurysms. Also, a peculiar syndrome where use of the arms and legs leads to cramping because of insufficient blood flow (limb claudication) can occur. PMR is diagnosed when the clinical picture is present along with elevated markers of inflammation (ESR and/or CRP). If temporal arteritis is suspected (headache, vision changes, limb claudication), biopsy of a temporal artery may be necessary to demonstrate inflammation of blood vessels.

PMR and TA can present with symmetric inflammatory arthritis similar to RA. These diseases can usually be distinguished by blood testing. In addition, headaches, vision changes, and large muscle pain are uncommon in RA, and if these are present, PMR and/or TA should be considered.

In part 2 of this article, I will discuss infectious diseases that need to be considered in the differential diagnosis of rheumatoid arthritis. When RA is suspected, it is critical to consult with an expert rheumatologist.

Costochondritis is a medical condition in which there is severe or moderate type of pain which is localised to the chest and usually seen on the rib joints. Ayurvedic herbal treatment for costochondritis is aimed at giving anti-inflammatory medicines to treat the basic disorder, control pain and tenderness, and also strengthen the costo-chondral joints, so as to avoid recurrence of this situation. Treatment is given in different stages, with control of pain and inflammation forming the first stage. This can be done with oral medication and supplemented by local application of medicines in the form of ointments, pastes, and medicated oils. Local application is usually followed by steam fomentation using herbal decoctions. The application of anti-inflammatory herbal pastes also provides long-term relief from pain and tenderness.

Stress, depression, generalized weakness and long-term fatigue are some of the factors responsible for this situation. The specific cause needs to be identified and treated with specific medicines. In addition, medicines may also be required to treat depression, stress, and weakness. Herbal medicines which have an immunomodulatory action and which provide long-term strength and vitality to the body are especially useful in this condition.

Such individuals need to develop a daily schedule of regular exercise, relaxation techniques, eat healthy food regularly at prescribed times, and obtain adequate rest and sleep. How To Treat And Prevent Wrist Pain


The wrist is an unusual joint because stiffness or even fusion causes relatively little difficulty, while if it is wobbly and unstable there can't be real problems. The wrist provides the platform from which the fine motions of the fingers operate. It is essential that this platform be stable. The eight wrist bones form a rather crude joint that is very limited in motion compared with, for example, the shoulder, but which is strong, and stable. Almost no regular human activities require the wrist to be bent all the way back or all the way forward, and the fingers don't operate as well as when the wrist is fully flexed or fully extended.

The wrist platform works best when the wrist is bent upward just a little. When the wrist is in proper position, you can pull or push most efficiently. To illustrate this position, make a fist and put your thumb in the middle of the wrist. Looking down your arm, the thumb should be on an imaginary horizontal line going straight down the middle of your forearm.

The wrist is very frequently involved in rheumatoid arthritis, and the side of the wrist by the thumb is very commonly involved in osteoarthritis.

The "carpal tunnel syndrome" can cause pain at the wrist. In this syndrome, the median nerve is trapped and squeezed as it passes through the fibrous carpal tunnel in the front of the wrist. Usually the squeezing results from too much inflammatory tissue. The cause can be tennis playing, a blow to the front of the wrist, canoe paddling, rheumatoid arthritis, or many other activities that repeatedly flex and extend the wrist. In addition to wrist pain, this syndrome can cause pains to shoot down into the fingers or up into the forearm; usually there is a numb feeling in the fingers, as if they were asleep.

You can diagnose this syndrome pretty well yourself. The numbness in the fingers will not involve the little finger and often will not involve the half of the ring finger nearest the little finger. If you tap with a finger on the front of the wrist, you may get a sudden tingling in the fingers similar to the feeling of hitting your funny bone. Tingling and pain in the carpal tunnel syndrome may be worse at right or when the wrists are bent down.

The key to management of wrist pain is splinting. Since stability is essential and loss of motion is not as serious in the wrist as in other joints, the treatment strategy is a little different. Exercises to stretch the joint are not very important. The strategy is to rest the joint in the position of best function. Wrist splints are available at hospital supply stores and some drugstores. Any that fit you are probably all right. The splint will be of plastic or aluminum and the hand rest will cock your wrist back just a bit. You can put a cloth sleeve around the splint to make it more comfortable against your skin and wrap the splint on your arm gently with an elastic bandage to keep it in place. That's all there is to it. Wear it all the time for a few days, then just at night for a few weeks. This simple treatment is all that is required for most wrist flare-ups. Even the carpal tunnel syndrome is initially treated by splinting. But since nerve damage is potentially serious, give your doctor a call if you seem to have the carpal tunnel syndrome.

No major pain medication should be necessary. Aspirin and similar-strength medications are all right but probably won't help very much. If you are taking a prescribed anti-inflammatory drug be certain that you are taking it just as directed; sometimes a flare-up is simply due to inadequate medication. If you know what triggered the pain, work out a way to avoid that activity. Common sense means listen to the pain message.

Arthritis - Signs and Symptoms

Arthritis Problems?


There are over 100 different types of arthritis. The two most commonly related to age are osteoarthritis and rheumatoid arthritis. The two types of arthritis, although the same family of disease, have different progression in the body.

Osteoarthritis is a degenerative joint disease. This is a chronic condition causing the breakdown of the joints' cartilage. As the cartilage breaks down, the joints then rub bone on bone and can cause pain, restricted movement and stiffness.

Rheumatoid Arthritis on the other hand is an autoimmune disease. This commonly effects the smaller joints such as those of the wrist, feet, knees, ankles and hands. It can also cause pain from moderate to severe. The inflammation may in time effect internal organs as well. Increased loss of range of motion along with swelling, redness and stiffness.

Diagnosing these types of arthritis is in the hands of both you and your physician. When you go to the doctors office have a list of all of your medications, symptoms such as pain, swelling that causes asymmetry in the effected joints, fatigue and some redness.

The primary symptoms that indicate the possibility of one of these types of arthritis are:

  • swelling or fluid accumulation
  • tenderness or stiffness
  • warmth accompanied by redness around the joint
  • reduced range of motion
  • fever
  • fatigue

Osteoarthritis is the most common type. This will also cause limited motion, stiffness and joint pain. Usually it progresses to a necessary change in your daily activities due to that pain and stiffness. Osteoarthritis develops gradually. Effects are noticed later in the day after constant use. It does not effect other organs or have a systemic effect. Swelling, stiffness and warmth can also present themselves after lengthy inactivity. Spurs and enlargements of bones is also characteristic and osteoarthritis can appear along with rheumatoid arthritis.

An accurate diagnosing of these types of arthritis must be done by your physician. Your medical examination, your history and your account of symptoms along with progression are vitally important to your diagnosis.

There are many different types of arthritis and some diagnoses are difficult to ascertain. Blood tests and x-rays are usually used as part of an accurate diagnosis.

Getting diagnosed is the first part of the battle. Treatment then presents the second part of the battle.

Medications available for types of arthritis.

Medications fall into classes for both types of arthritis. These work to control symptoms, improve function and mobility and prevent joint damage. There are five basic categories of arthritis medication.

1. painkillers/analgesics

2. nonsteroidal anti-inflammatory drugs or NSAIDS

3. corticosteroids

4. disease-modifying anti-rheumatics DMARD

5. newer biologics.

It is important to learn about these medications and know your options for the types of arthritis you may have, either rheumatoid or osteoarthritis.

NSAIDS
This class of medication has been in use for over three decades. They contain both a fever-reducing and painkilling formula. The risks and benefits of NSAIDS need to be considered, as they do have some severe side effects in some patients.

ANALGESICS
Analgesics are pain killing drugs that block the signals to the brain or misinterprets those signals. There are narcotic type and non-narcotic types of analgesics. Neither will cause the patient to become unconscious or producing anesthetic effects.

STEROIDS
Steroids/corticosteroids are related to cortisol. The adrenal gland produces this hormone. While initially the use of steroids for arthritis was so impressive they considered it a cure. As time progressed and use expanded, many severe side effects became known. Steroids are still used either by injections or orally but only to decrease inflammation.

BIOLOGICS
This anti-rheumatic class of drugs either restore or stimulate the immune system. The immune systems' job is to fight off disease and infection. These drugs were specifically designed for rheumatoid arthritis.

DMARD
Disease-modifying drugs seem to decrease the inflammation associated with arthritis. It is however, not categorized as such. Instead of directly reducing pain, they modify the immune system, thus slowing the disease process.

NATURAL METHODS
As many people try to find alternative and thus more natural forms of treatment to complement the traditional types some get very little respect in the medical community. Before embarking on any remedies other than those prescribed by your physician, you should consult with them due to the fact that although "natural", they may interfere with the actions of your prescribed medications.

PAIN RELIEVERS
Both non-pharmacologic and drug pain relievers are available. Due to the fact that chronic pain is debilitating and will interfere with your normal daily activities it is necessary to find the correct way to manage that pain. Before trying anything new, please consult with your physician. Some patients have found that relaxation therapies may help reduce stress and thus relieve some of the discomfort caused by the types of arthritis. Although painkillers may help in the earlier stages of the both types of arthritis, it is no guarantee that it will help throughout the entire disease process.

JOINT REPLACEMENT
Although surgical intervention sounds a bit extreme, it has proven to be very successful. Joint replacement surgery removes joints that are so severely affected that the patient has no other recourse. Before having your joint replacement consult with your health care team about what to expect. Following their advice will get you through the procedure and lead you to a much freer life. There are great similarities between both types of arthritis, rheumatoid and osteoarthritis. Pain, swelling, redness, stiffness are evident in both. This is not as hopeless as it was decades ago when someone in your parents family became greatly debilitated by either type. Main Causes of Upper Back Pain


Upper back pain can be caused by muscular or joint dysfunction. Each vertebrae of the thoracic spine connects to ribs on either side. This segment of the spine is less mobile than the lumbar and cervical (neck) regions, as its main function is to provide stability and structural support to the vital organs within the ribcage. Since it is not mobile, disc injuries are uncommon in the thoracic spine.

Muscular Causes of Pain

The rhomboid and trapezius muscles in the upper back stretch between the spine and shoulder blades. They are large muscles that provide support to the spine and are used in many daily actions, especially pulling motions. These muscles may become irritated due to either weakness or overuse.

If the large upper back muscles are not strong, they can be strained by simple daily activities. If core muscles in the stomach and lower back are weak, your posture will suffer since the upper body's weight will not be adequately supported. This leads the shoulders to droop and hunch over, pulling on the muscles in the upper back. Proper conditioning of muscles throughout the back and core is necessary to avoid and correct this type of muscular pain. If you have upper back pain, a physical therapist may be needed to instruct you on the types of exercise that will benefit you.

Upper back muscles can become strained by overuse. Jobs that require repetitive arm motions can overtax your muscles, as can rigorous sports like rowing. Muscles that are chronically tense may develop trigger points, dense knots that form in the tissue surrounding the muscle called myofascia. These knots cause localized pain and make it difficult for muscles to relax. They can also refer pain to other parts of the body by pressing on nerves. Trigger points and overuse injuries are best treated with massage therapy. Deep tissue and trigger point massage will force your muscles to relax. Physical therapy may be needed to correct poor body mechanics that cause injuries.

Joint Causes of Pain

Thoracic vertebrae each have a number of joints: two on each side that link to other vertebrae called facet joints and one on each side that connects the vertebra to a rib, called costovertebral joints. Unlike the rest of the body's joints, thoracic joints usually do not wear down due to the normal aging process, since the area is very stable. There are other situations that can cause joint dysfunction in the upper back.

Facet and costovertebral joint dysfunction generally occur in the thoracic spine due to poor body mechanics. Excessive bending, twisting and lifting can stress the joints, damaging the strong connective tissue that comprises joints. The joints can become inflamed and stiff, limiting range of motion of the shoulder and causing upper back pain. Costovertebral joint dysfunction can cause pain both in the back and the ribcage.

Chiropractic manipulation is considered the best conservative treatment for thoracic joint dysfunction. Chiropractors work to restore range of motion and joint alignment.

There are a number of conservative treatments for upper back pain management. Massage, chiropractic care and physical therapy can help to reduce muscular pain and resolve joint dysfunction. Make sure you are performing your daily tasks with proper body mechanics and practicing good posture; these will help to prevent upper back pain in the future.

Beat Eye Disease - Aging People Forced To Adjust To an Eye Disease That's Related To Aging


There might be a way to beat eye disease. Aging people are forced to adjust to macular degeneration (MD), which is an eye disease that's related to aging. This commentary talks about how people, who are going blind, have to adjust to their loss of vision; and it introduces a potential solution to the problem.

Aging people start losing their eyesight because neurons are affected. When neural input is disrupted, the brain can't discern visual input because the part of the eye related to the disease, the macula, becomes dysfunctional.

Massachusetts Institute of Technology (MIT) researchers, in a study published in the March 4, 2009 issue of Journal of Neuroscience; said they discovered that aging people must adjust to loss of vision with a unique technique, in order to see better; and the researchers related it to the way their brains process visual input.

Neurons apparently need to receive input. So, when usual visual information of aging people is disrupted, they are forced to respond with "the next best thing," according to a press release of MIT McGovern Institute for Brain Research; by senior author Nancy Kanwisher.

Researchers discovered that when cells in the fovea, which is the part of the retina that's responsible for a person's central field of vision and it's the central part of the macular portion of the eye, degenerate; the brain forces undamaged neurons, attached to the degenerated section of the fovea on outer portions of the macula, to respond to stimuli by necessitating aging people with MD to tilt their heads.

The MIT researchers theorized that it's a kind of internal reorganization, and the brain forces aging people to adjust so their brains can interpret the visual map of the eye; rather than using the outer part of the eye, known as the cortex, to get information from other neurons.

Kanwisher said their study demonstrated that discernible changes in neural responses of aging people suffering from MD are probably forced by their brains, because of "the lack of input to a population of neurons," to adjust; rather than their brains changing the discernible visual information-processing method.

Macular degeneration is the most common disease related to adult blindness, and almost 2 million people are affected by it in the United States alone. Their brains force people to roll their eyes upward, so they can adjust to the lack of central vision. They exploit the preferred retinal locus (PRL), which is the undamaged area adjacent to and underneath the part of the retina affected by MD, by tilting the head.

Kanwisher said that MD is a great opportunity to understand more about flexibility in the eye cortex in aging adults. But it might be possible to beat the eye disease with simple eye drops.

The theory is that macular degeneration is linked to hardening of the lens in the eye, which creates the "lack of input to a population of neurons" that the MIT researchers mentioned in their study; and those neurons in the fovea of the macula, over time, degenerate. In other words, "If you don't use it, you lose it."

The brain must get visual input by any mean that it can get it. If lenses of the eyes become stiff and cloudy, thereby obstructing the necessary data that the brain requires; neurons of the fovea degenerate.

The brain forces people to tilt their heads to get the visual input that their brains desperately need, by using neurons adjacent to and underneath the neurons that are degenerated in the fovea, by accessing visual information through the cortex of the eye rather than trying to analyze data through ineffective eyeball lenses.

It's a horrible disadvantage to have to tilt the head just to get a blurry image. But people do what people must do; and people simply must be able to see, so their brains demand them to get visual information by whatever means is available.

There is a formula that has been tested for 10 years, which not only may impede macular degeneration and subsequent blindness; but it might beat presbyopia and MD altogether IF people use it early enough, and IF aging people have just enough faith to try it.

Genetic factors are thought to be involved in about fifty percent of osteoarthritis cases in the hands and hips and a somewhat lower percentage of cases for the knee. It's known to play a role in a systemic form of arthritis - rheumatoid arthritis and other muscle and joint issues associated with autoimmune diseases. For part of a dietary approach try avoiding seven of the eight most allergic foods - wheat, which is the most allergenic of all, corn, eggs, milk, peanuts, fish and shellfish. So the first step in managing your arthritis is to change your diet. I think all health professionals would agree with this.

This is the ideal alignment for your leg. A word about your hip joint: did you know that your "hips" are not the wide bones of your pelvis, but the place where you crease when you bend your knee towards your chest? This is where your heel should line up when your foot hits the ground when walking. The hip joint itself is really quite narrow - about the same distance apart as your sits bones in the back.

3. When you stand, walk and exercise, the alignment of your whole leg affects which muscles you use to move. And the way you use your muscles strongly influences your alignment. Be very aware of keeping correct alignment from your pelvis to your foot - hip to 2nd toe - so that you create good balance around your knees. If you do exercises like squats and leg press, do you pay attention to your leg alignment? Your knee program will be far more effective if you do. Remember that pain is your body's way of telling you that something's not right, so if you can't modify your position to relieve the pain, skip the exercise. Remember also that not every "recommended" knee exercise is right for every knee - you need to find out what's right for your knees.

The point is that your knee pain is usually just a result of the alignment you set up from the ground (you feet and ankles) and the load you put on them from above (your hips, pelvis and trunk). Figure out how to de-stress your knee joints by changing your whole-body alignment and your knee pain and stiffness will resolve.

TMJ Disorder - What is it and How Your Dentist Can Help


The temporomandibular joint (TMJ) is the joint located on each side of your head in front of your ears, where your lower jawbone meets your skull. This important joint enables you to eat, speak and yawn, and in order to function properly, each intricate part of the joint must work together properly. Any problem that prevents the TMJ's complex system from functioning properly can result in TMJ disorder, with symptoms including loss of jaw movement and intense jaw pain.

Surgery for Temporomandibular Joint Disorder


Surgery is a permanent choice and should only be used as a last resort. Make sure you have tried all the other alternatives before making this final move. Surgery for Temporomandibular Joint Disorder, or TMJ, is rarely used for a treatment because it does not guarantee a cure and could further damage the joint. Surgery may also decrease your jaw's range of movement as the jaw heals with scar tissue, which is harder and tighter than normal tissue. Surgery may become an option if you have tried everything and are still in constant, intense pain that is disrupting your ability to eat or work and is disabling your every day life. The other time that surgery may help is if a specific, severe structural problem has been definitely diagnosed.

Arthroscopy, arthrocentesis, and open-joint surgery are the three types of surgery for this disorder. What type to have depends on the type of problems or severity of the temporomandibular joint pain.

All the surgeries are done with general anesthesia. Make sure you do not have difficulties with using anesthesia. Also, make sure to check the qualifications of your surgeon and anesthesiologist..

Arthroscopy - First general anesthesia is given. Then a small incision is made by the surgeon in front of the ear. This incision allows a tiny, thin instrument with a lens and a light to be inserted. This device is hooked to a video screen where the surgeon examines the temporomandibular joint and areas around it. Removal of inflamed tissue or realignment of a disc or condyle is done by what the surgeon can see. The condyle is the part of your jaw consisting of the "ball" portion of the "ball and socket."

This choice is considered less invasive than open-joint surgery. Arthroscopic surgery leaves less scarring, usually has less complications and a shorter recovery period. The cause of the TMJ may rule out this option for you and open-joint surgery may become necessary.

Open-joint Surgery - For this option a general anesthesia is given. The incision is larger than with arthroscopy as the entire area around the temporomandibular joint needs to be opened for a full view by the surgeon. The larger incision is also to allow better access. The type of open-joint surgery needed depends on what is wrong. It can be that there is extreme scarring or chips of bone in the joint that need to be adjusted. The surgeon may find a tumor that will need to be removed,. The worst cases involve the deteriorating bony structures of the jaw and will need to be reconstructed. With the more intense problems and surgeries comes the longer healing times and chances of permanent scarring and nerve trauma.

Arthrocentesis - This is a minor procedure for sudden-onset problems when restricted jaw opening is diagnosed. It is performed under general anesthesia. This choice is for cases with no major history of temporomandibular joint disorders. The affected joints have a needle inserted and are washed out with sterile fluids. If necessary, insertion of another instrument to remove tissue adhesion or dislodge a stuck disc in the condyle is done at the same time. This is a common procedure for a painfully locked jaw and is successful in 94% of cases.

Foot Swelling - Good, Bad, and Ugly


Hot air balloons are beautiful, aren't they? Bright and colorful, they drift quietly through the air, removing the traveler from toil and endless trouble. Unfortunately, not all things that swell up like hot air balloons are beautiful or free from difficulty. Joints, for one, are decidedly less pleasant when they begin to swell. Fortunately, your friendly local podiatrist has the know how to help reduce swelling and get your joints to function a bit better.

Swelling is, of course, perfectly natural. It's the body's way of responding to injury or infection. After damage to a joint, more blood is sent to the area to try to heal it. Unfortunately, the increase of blood also causes a buildup of fluid in the tissues around the joint, which tends to cause pressure and pain.

Swelling may be caused by many things. It may be a result of trauma, such as an ankle sprain or break, or stubbing your toe. Or, swelling might accompany arthritis, which itself may be caused by crystal deposits in the joint (in the case of gout), degeneration of cartilage in the joint (osteoarthritis), an autoimmune disease attacking the joints (rheumatoid arthritis), and so on. Or, joint swelling may be caused by an infection.

Swelling may appear as one of the first symptoms of arthritis. Those who suffer from rheumatoid arthritis, for instance, often list joint swelling as one of the earliest symptoms. In the case of gout, swelling may be so severe that the skin over the joint will appear stretched and shiny.

Joint swelling is usually accompanied by other symptoms, such as redness, warmth, stiffness in the joint, and pain. You may also find it difficult to walk, usually because of pain, but sometimes because your joint has lost some mobility.

Because the potential causes of joint swelling are so numerous, you should be prepared to give plenty of information to your podiatrist so he or she can make an accurate diagnosis. Your foot doctor will most likely perform a physical examination of your foot, check your joint for range of motion, and will inquire about your medical history and a history of your symptoms. Be prepared to answer questions such as how long the swelling has been present, whether it comes and goes, when it's worst (i.e. is it worse at night, or perhaps does it tend to get worse during certain activities such as running), and also whether or not you've ever ridden in a hot air balloon. (Actually, they may not ask that last question.)

There are a few tests your podiatrist may suggest to determine the cause of your joint swelling. These may include X-rays, CT scans, or MRIs to get an accurate picture of what's going on inside your joint. Additionally, your doctor may want to withdraw some fluid from your joint to test it for infection, or for uric acid crystals in the case of gout.

Treating swelling in the joint generally focuses on reducing bloodflow to the area. This may be accomplished by icing the joint (20 minutes on over a thin towel and 40 minutes off), and elevating your foot at hip level, or slightly higher. Your podiatrist may also recommend anti-inflammatory medications such as ibuprofen, or sometimes cortisone injections.

Of course, your doctor will also want to address the underlying injury or problem that's actually causing the swelling. This may be done by using casts, braces, or orthotics (prescription shoe inserts) to provide support to the foot and prevent further injury. Physical therapy might be used to help strengthen your muscles. In some cases, surgery is necessary to reduce swelling and reestablish the functionality of the joint, or your foot surgeon may opt to fuse the joint to reduce pain and swelling.

Some patients also find it beneficial to soar through the sky in a basket suspended from a balloon filled with hot air. Actually, this may not really be the case. But hot air balloon rides (or even just looking at pictures of hot air balloons) will help you remember that, even though your joints may be painful and unpleasant when they swell up, some swollen things are actually pretty darn nice.

GSP Rushfit: How It Keeps Me Fit With Less Joint Pain


Like some people over 40, I am developing joint pain. I am also a fitness fanatic. These two characteristics can work well together or be disastrous. After going through other, similar workout regimens, I had huge health gains but pretty severe joint pain. It was beyond "no pain, no gain" stuff -- this was full-on pain in the bone. So, when I discovered Georges St. Pierre (GSP), one of the best MMA (mixed-martial arts) fighters in the world, had just released a home workout regimen on DVD, I had to check it out. I was, of course, a bit skeptical and concerned, as GSP is not a very talkative guy and I was still a bit sore from my last cycle of another regimen. I was not disappointed, though, as GSP Rushfit is not as hard on the joints as my previous workout cycle. GSP is also a fairly colorful guy, as well, which made the DVDs much more watchable than I thought they would be.

The GSP Rushfit package itself is a fairly comprehensive kit. It includes the main course on several DVDs, along with bonus DVDs with extra workouts for full-body conditioning and one for balance and agility. Also included are three training calendars -- one for the beginner, medium and expert, and extra guides with workout tips and nutritional information. Seven workouts are included in total, each lasting around 50 minutes (including warm-up and cool-down). The package is obviously professionally done, so there are no complaints about book or video quality.

One may be surprised that GSP does not actually lead the exercises. His trainer, Erik Owings, conducts the actual workouts while Georges participates alongside. Erik is the more vocal of the two, with GSP contributing color commentary when appropriate. Erik Owings is an accomplished MMA fighter himself, so the workouts are certainly MMA-themed. You certainly get a "fighting focus" with these routines, and Erik does an adequate job of transforming mundane exercises into fighting techniques. This is not, of course, a self-defense course, but it's certainly MMA-themed. Most men, and plenty of ladies, will still enjoy this thoroughly, as it breaks the monotony of other workout routines.

GSP Rushfit's main advantage in this genre is that it's a bit less harsh on the joints. I found that after my cycle of GSP Rushfit I was not in as much pain as I would be after a run-through of other multi-week fitness plans. My knees and elbows were not banged up, and I felt that GSP Rushfit's focus on stretching and joint warm-ups eased the pressure on my joints. This was a huge benefit for me and it's the reason why I will be putting the other DVD series on the shelf for a long time.

GSP Rushfit has been a very entertaining and effective method for my home-based workout regimen. It's easy on my joints but keeps intensity high, and for that reason, I'm sticking with it.
